The mom deserves to know so she can protect her baby. She should be 
making at least an ounce per breast per feeding on day 5, not drops. And 
her baby should be stooling. You can tell her you've heard of a 
condition where moms have less milk making tissue than optimal, and it's 
called breast hypoplasia. Then let her do the research. You can also 
explain what's normal intake for her baby, and encourage her to FEED THE 
BABY. I'd encourage her to start trying a supplementer at breast, it 
should be easier to work with now than when her baby gets into trouble 
from insufficient energy intake.


 I agree with Catherine. Someone needs to tell this mom--I have a feeling she will be both angry that no one told her before and relieved to know that there is an explanation for her experiences. You are not making a diagnosis when you give information to someone. Quite the opposite--I think there is nothing more unethical than withholding information. I would not rely on the midwife to relay info, but I would encourage the mother to discuss the situation with her midwife.? To add to the serendipity of the thread--I also saw a mom this week with apparent hypoplasia. She had seen an LC in the hospital as well as one in private practice. Neither ever mentioned the possibility at all. (I saw the notes from the 2nd LC--no mention and treatment plan was not appropriate for hypoplasia). Mom cried for the first hour of our appt. Hopefully, we are on track to an optimized situation now. She has already increased her supply in just a couple of days with a change in pump, change in latch, CST and galactogogues. Fortunately, she has a very supportive ped.
