With respect ..... the study sounds like it's 
been funded by the manufacturers of these little 
pots of Activia or something similar, and they 
would prefer mothers to use this product rather 
than the usual treatment of breast pain/mastitis 
with antibiotics....  Wouldn't you usually do a 
milk culture and prescribe an appropriate 
antibiotic?  I've worked with a series of mothers 
who had the symptoms you describe, and the lab 
turned up staph pyogenes.  It sometimes took two 
course of Abx to fix the problem and sometimes 
the ONLY symptoms were the pain (stabbling, like 
needles) or slightly pink but extremely sore 
nipples (mothers described felt like ground 
glass), no abrasions.  But lactobacilli????

The other frequent referral to sub-acute mastitis 
was in connection with HIV and breastfeeding, at 
the height of the time when breastfeeding was 
being discouraged by HIV+ mothers and when UNICEF 
was trying hard - and succeeding - in pushing 
formula.  Three studies done by Richard Semba and 
colleagues in Malawi in about 1999.  Sub-acute 
mastitis was inferred due to elevated sodium 
content of the milk.  As we know, sodium levels 
do rise with mastitis.  Or with inadequate breast 
drainage.  But what wasn't known then was that 
mixed feeding increases the risk of HIV 
transmission.  so I'm convinced that these 
mothers were either mixed feeding or partly 
weaning their babies .....they researchers said 
they were exclusively breastfeeding, but we knew 
that no mothers were advised to exclusively 
breastfeed in Malawi at that time, and that in 
fact exclusive breastfeeding is not often 
practised in African societies (until now..... 
now that it's being highly recommended...). but 
the diagnosis of sub-acute mastitis became fashionable at that point....

Sorry to go on about this, but it is a really big problem, lots of mothers
spending a lot of money and not getting any real help.

SUBACUTE MASTITIS

The term subacute mastitis I have only found referring to bovine mastits
EXCEPT in papers written by Arroyo et al (the Spanish lactobacilli group).
They say "subacute mastitis in lactating women is caused by
coagulase-negative
Staphylococcus (epidermidis), Streptococcus viridans/mitis and some
species of Corynebacterium. These women have local pain, more or less
intense, like stabbing needles, cramps or burning, with no visible
inflamation and no systemic symptoms. They do not state how long "subacute"
means... I undestand it´s longer than acute and shorter than chronic, but
they define no timeline.
They then go on to state that subacute mastitis should be treated with
lactobacilli. They also state that there are two clinical trials that back
up this statement; the clinical trials are their own, and the women enrolled
in those two clinical trials had acute mastitis symptoms.

Subacute mastitis has become an epidemic in Spain. Every woman with
shooting pains is receiving (or rather, spending money on) lactobacilli.
With or without previous milk culture, and usually almost always regarless
of the result of the culture. SOme women are now being advised to take
lactobacilli DURING PREGNANCY to prevent breast problems.

QUESTION: does subacute mastitis of the lactating breast exist in any other
country other than Spain? Are you living a probiotic campaign that is not
supported by much actual research?
