Get that kid to a another Pediatrician or a Pediatric Dermatologist!
Could be a impaired lymphatic drainage, impaired venous return, or a deep
hemangioma.  Could also be simply a "birth defect" due to uneven perfusion of
the extremity during fetal development.  If it has persisted for 4 weeks, it
needs to be investigated and explained!
