Another thought is oral weakness or disorganization.  When I read, "Baby has a very ineffective latch, spills milk and can be detached easily," I think low muscle tone; refer to SLP or OT.   Ditto for baby nursing effectively only on one side, only in one position.  Sometimes the exact position can be duplicated on the other side, but finding the underlying weakness (and addressing it) is crucial.  If one side of a baby's mouth is much weaker than the other, positioning is difficult.   Baby might have reflux or other problems, but if the oral muscles are not working in unison, baby is probably taking in lots of air.  Also, if it is an oral motor issue, adding flow at the breast may make breastfeeding more difficult for mom and baby since the underlying weakness hasn't been corrected.
