The WHO statements are pretty clear to me - don't use it in infants under 2
months or with those at highest risk if at all possible!  So - in the case
of those who do not have access to human milk, or ready to feed formula then
the next action would be powdered infant formula BUT it MUST be prepared
PROPERLY!  And I believe it behooves those of us who work with parents who
will be using PIF to do so - including hospitals.  I believe that, in this
day and age, if we (HCPs) are able to give a simple handout and talk with
the parents and explain WHY they need to prepare with water that is hot and
not tap water right out of the tap (your baby's life may depend on it!) then
it reduces our risk of responsibility if something happens to baby.  (Here
is a copy of the handout I gave them, here in my chart you can see I spent x
amount of time discussing safe preparation.)  And also, what about informed
consent??  I cannot tell you how many moms were given a can of PIF and not
told how to use it - just TO use is - and are PISSED OFF because they had NO
CLUE they were putting their baby's life/health at risk by not prepping it
safely.
