Just saw a mom tonight with an unusual presentation. 2 wks postpartum, fever
of 105, reports engorged breast, achiness, nipple latch pain, sore breast,
decreased supply, but no real red wedge; instead, she looks "sunburned"
(honest, I thought she'd been in the sun till she told me it just appeared)
from neck to pubic area. It has also become itchy-- no medications have
triggered it. (fever fell to 101 with tylenol while I was there, and mom has
been started on dicloxacillin)

On top of this, one breast has a "white" area of about 1.5" diameter on
lower side, below incision area. It looks blanched, with a small red
dot/blotch in the middle (sort of like a mole or supernumerary nipple, but
just not quite right). My first sight impression was "abscess", but there is
nothing palpable there whatsoever. Dad is a new MD and had already thought
of the same thing and felt nothing, either.

Hx-- mom had implants 3 yrs ago, peri-areolar incision. Baby has beautiful
mouth, opens wide-- but there have been compression stripes. Weight is 1#
below BW today. With improved positioning, baby took 1oz total from both
breasts, and we could only pump 10ccs extra. Most mom has ever pumped (with
First Years pump) was 2.5-3oz.

I'm suspecting mechanical blockages, but the red sunburn rash and the white
blanched area have me stymied. Anyone else ever seen these associated with
mastitis?
