I need Phone number or fax#  Dr Labbok is affiliated with?

I need her e-mail address as many Doctors i hang with would like a profile
on Dr Labbok like mission statements, policies, opinions etc before they
feel fully able to send letters in her behalf for the surgeon general post,
i was unable to answer all their questions regarding her philosophy or her
list of credentials.  If only they could meet her she's an extraordinary
person.......
