a couple of slightly radical ideas: (which perhaps you are already doing)

1) assume they are still breastfeeding instead of asking if they are at the
first appt (perhaps you already do this) this plants the subtle seed that
you expect them to breastfeed for awhile
2) state the AAP position on breastfeeding to a year casually and early on
3) speak in terms of the risks of formula feeding in the same way you would
speak of the risks of smoking around their babies.

It's so nice to see a doctor who wants to increase breastfeeding duration!
I'm so used to all the doctors in my area. At my 2nd child's 2 year appt, I
was asked what milk she drank. I replied "breast milk". Dr says "you mean
she's still breastfeeding?" Well, yes that is the source of breastmilk! :-)
