Good morning from the land of the dreadful head cold.

Any mother who has experienced a uterine rupture while
pregnant or laboring has been through a life-
threatening emergency. Please affirm with these moms
that they have been through an unusually traumatic
physical event. If a hysterectomy was necessary, they
may also need to grieve the loss of a "normal" labor,
birth, and post- birth experience; they may also be
grieving for their capability to bear other children,
which has been lost, and in a very sudden way.

I would not expect an abundant milk supply in the
early weeks from these moms. They need time to heal
physically and emotionally. Beating themselves up
because of dwindling or poor supplies is the product
of unrealistic expectations and needs to be addressed
in a loving way. This is not to say that these moms
shouldn't be pumping or trying to boost their
supplies; but I do think it helps to let them know
that their labor complications and needed recovery
time are a factor.

Carole Jernigan, IBCLC
and veteran of 8 years in the trenches of high-risk OB
(I still miss it sometimes!)
