I have finally completed "Breastfeeding the Hospitalized Baby".  It is

It can be freely copied and reformatted to usable hard copy.  I wasn't able to
retain the "pamphlet" formatting while publishing it to the web, so it prints
out as one long document.  Sorry about that.
  It is aimed at the mother, with some helpful tips for successfully
establishing breastfeeding with the full-term hospitalized baby.  I haven't
yet read the new LLL info on this topic, so I don't know how it compares.
