Gilbert's Syndrome is a common genetic disorder that causes reduced 
activity of the primary enzyme that conjugates bilirubin, which normally 
allows  it to pass into the intestines for excretion. Uncongugated 
bilirubin can build up higher than normal levels whenever there is 
greater production of bilirubin, such as in the newborn period when 
fetal hemoglobin is being recycled into bilirubin,  during excessive 
breakdown of red blood cells (hemolysis) or excessive production of 
bilirubin (illness).  Other than in infancy when it is a major cause of 
jaundice, it is rarely a problem in adulthood, except at times of 
metabolic stress. Starvation and illness increase bilirubin production, 
and may allow an adult with Gilbert's to become jaundiced, hence the 
need to eat well.
I'm pretty sure uncongugated bilirubin does not pass into milk, since 
it's insoluble. So even if the donor with Gilbert's syndrome does become 
jaundiced, her milk should be perfectly safe. She'll need to make sure 
to consume the calories she expends in producing extra milk to avoid 
impacting her own health.
