I think parents do have to go home with guidelines as to how much urine a
baby should make.  It was lack of that kind of knowlege that led to the
horror stories presented on 20-20 (or was it prime time) with the babies who
were 20% dehydrated and had horrible compications. Well this would never have
happened if the mothers had noticed decreased urine output that happens
around 10% dehydration. YOu can't get to 20 without passing ten.
The problem is not the guideline, it is what you do with the information. No
urine outpute as per mom, DOES NOT MEAN AUTOMATIC FORMULA. It should mean
automatic prompt assessment by someone well versed in lactation management.
SOmestines this can be the pediatriciena, and sometimes, unfortunately it
can't. Sure if the baby is otherwise well he can be followed with close
observation or even supplemented with expressed breastmilk.  However, the
decision that the baby is otherwise well should be done by someone with
experience is assesssing newborns, not a first time mother.
