I am assuming this "fissure" is at the base of the nipple?   We have
found it very useful to have these moms wear a shell between
feedings/pumpings to hold the edges of the skin apart, so the wound can
granulate in from the base.  It can take a few weeks depending on the
depth of the wound.  I would also use antibiotic ointment.  Some of
these require oral antibiotics...and some are caused by yeast making the
skin more "friable" and susceptible to tearing apart.
