My own expereince and thoughts:

My primarily breastfed premie (32 weeks gestation) was seriously iron
deficient at ten months, he was nearly polycythemic at birth ~ borderline. I
tried supplementing with Floradix then went to conventional iron rx and his
iron levels rose immediately. He is now four years old, has not had issues
with anemia since. This was his first supplement to breastmilk and food
since birth.

When something is amiss with premies in utero, the baby's body responds by
loading up on red blood cells to carry the oxygen, protect the baby. At
birth, we then see jaundice which can take several weeks to resolve in
premies because there is such a huge load of bilirubin to process by this
little liver that is  not yet fully developed  at term. Phototherapy is used
to bring the bilirubin levels down so we do not know what would happen if
left to do what nature planned.

Then the slide continues, down down down goes the iron level over the months
that follow. Had they been left high, would the iron values be normal? Is
this the normal pysiologic process?

I doubt there is any study out there of what is the normal physiologic
process in this scenario because so many premies have concentrated ABM with
vitamins and minerals fortifier added to their breastmilk feedings.
