Question to Nancy Wight:  in Europe where I've read they just increase the amount of breast milk given to preemies rather than fortify - do they have higher incidence of problems because of increased volumes?  If you increase protein by increasing volumes, are there other problems?  When you try to modify milks to make them different from naturally produced milk, are there no long-range consequences?

Almost always, poor growth in premies is due to lack of sufficient protein,  not calories.  "Hindmilk" is identical in protein to "foremilk".
