Perhaps a punch biopsy would be helpful in determining the cause?

The skin on the bump is much lighter than the surrounding skin (bump is pink, nipple and areola are brown). It never changes in size and is always painful to the touch, for example, when she puts a bra on. Baby is 10 days old, but the painful bump became raised and painful and the color changed during the first trimester of pregnancy. A nipple shield helps it to be less painful, but it sure would be nice to figure out the cause.
