Dear Readers,
I work in a hospital setting. We have always placed a sticker on any bottle of EBM and it is stored in a special refrigerator in the lactation room. The fridge is just for breastmilk. We have recently had all patient stickers that have a bar code on them just like a patient bracelet for safety. 
It has been brought to my attention that as a safety measure we should be using a barcoding and milk should be taken out of the fridge same as meds from the medcart. 
Most of the moms that are pumping are either pumping because their baby has been transferred to another facility or the mom is pumping because her milk is in and she is uncomfortable. 
Occasionally a mom may choose to pump only and bottle feed ebm. We have never had an issue of a baby receiving someone else's milk.
