first of all, breastmilk is a physiologic fluid. we should only look for
citations that show it is dangerous to aspirate breastmilk, not ones that
"prove" it is safe. when the doctor says "so and so is unsafe" then he needs
to provide references to show that that assertion is true. we should not be
"defending" breastfeeding when it is clear that the assumption is that
breastmilk is the equivalent of formula in this situation.

in other words, we know that aspirating formula is dangerous, just as it is
to aspirate any other NON physiologic fluid. even aspirating blood into your
lungs, even if it were your own, would probably be dangerous. however, is
aspirating your own saliva dangerous? now go from there with breastmilk.

i have heard at more than one talk about reflux that one of the reasons that
exclusively breastfed babies do so well with reflux (only in comparison with
formula fed babies, i don't mean that they are happy or not suffering) is
that aspirating breastmilk is not as painful or harmful as aspirating other
foods, and that the rate of pneumonia is far lower.

of course there are components in milk that differ from those in saliva,
perhaps they would be more likely to cause problems. but, since the original
question about this was regarding typical american circs, done while the baby
is only receiving colostrum, and the volume is generally low, it still should
not be a problem.

breastfeeding on trial again. prove it's safe!
