With my father, it was liver enzymes. He was alcoholic and his liver showed it. His MD made it clear that if he took another drink, his liver would show it long after the alcohol was gone from his system. 
Liver enzymes are detectible in a blood test.

We generally use M&MM & Dr. Hale's editorial comment in one of his newsletters as a guide for alcohol consumption during lactation. In the US, we also advise / strongly urge NO alcohol consumption during pregnancy.

I only know about these enzyme being able to be detected as I have a very 
sick friend (not breastfeeding) who has been regularly tested for alcohol in 
the lead up to surgery - she was under the impression the doctors wouldn't 
be able to detect anything if she had no alcohol for about 24 hours before 
the testing. BUT one time she did have a drink or two and it was 48 hours 
later the enzyme was still detected in her blood tests.

I believe in Australia our health authorities have recommended ZERO alcohol 
during pregnancy and breastfeeding.
