Thanks to you all for your very thoughtful responses. I appreciate them all! 

Spoke to the doctor's office today (he had the nurse return my call), confirmed that they'd gotten my communications, re-shared my findings and stressed the need for this mom to get referrals for the TT/ULT as well as support services (which are available to her in our community). Also communicated with the WIC peer counselor working with this mom, so we're all on the same page. Waiting to hear back from mom about meeting in the next few days to retune the plan and provide further education and support. 

You've given me some food for though on other areas to explore with this pair, and I thank you all again. As a newbie, I was/am feeling a bit out of my depth. I'm grateful for your experience and willingness to weigh in.
