I agree with what Rachel about this case.



In my experience, pediatricians are expecting babies to regain birthweight by 2 wks. If a baby is 3 oz under at 10 days, they are not too concerned. In my experience, pediatricians are not thinking about breastfeeding details and fairly often recommend a glycerin suppository to get baby to poop. Sometimes I think office nurses take these calls, and I wonder if the call ever gets to the dr.



Therefore, it is the breastfeeding specialist's job to sort out the breastfeeding. The lack of stooling and the lack of gain are red flags. Mother needs in person help and frequent weight checks til all is well. She is at high risk of low production, or perhaps she has a physiologic reason for primary low production. In either case, she needs in person help and followup. You will have to delicately discuss this with mother, that the baby is not in any serious danger at the moment, and that seems to be why dr. is not concerned, but that you know the importance of those early days as far as breastfeeding goes, and that is why she needs intervention from a bf specialist.



Sometimes mothers supplement, even a little, and if they tell the dr or nurse about that, that person might not worry and say "well, she's supplementing so it'll be ok" but....

we know that this doesn't fix the BREASTFEEDING problem, and she might well be supplementing too little, too much, or unnecessarily.
