Jan, I guess what I was trying to say is that the Kramer study helped to 
establish that BFHI is a programme which will ensure women receive best 
breastfeedign support -- when the programme was devised there was evidence 
about each of the steps, but no evidence about the whole programme and the 
BFI method of evaluating care practices.  So *after* PROBIT we can say with 
confidence that practicing to BFHI standards provides best practice 
standards of care for breastfeeding support.
