Ellen asks about nipple reconstruction after a burn.  I think the
same factors would apply as for any surgery involving the nipples.
As I understand it, the most important factor is whether the
innervation to the nipple is still intact.  Since nipple stimulation
is critical in the ongoing production of prolactin and oxytocin
(after the hormone changes with expulsion of the placenta starts the
whole process), anything that interrupts the nerve pathway can
adversely affect mom's ability to continue to produce and deliver
the milk.
Secondary would be the interruption of the ductal pathways.  I say
secondary because many of us have noted moms able to express at
least some milk even when the ducts have been totally interrupted.
There does seem to be some spontaneous reconnection occuring.  An
additional factor with burns is that there may be areas of scar
tissue that can block the milk drainage.  As with any breast injury
or surgery, try it, keep close watch on diaper output and weight
status, and supplement if needed to insure infant health.
