I had to deal with a question like this about half a year ago. One of my students in the lactation course I co-lecture was 3rd time pregnant during course and gave birth to her son sometime around the july-exams (she passed). She had had supply issues with the first. In learning during course she was getting more and more determined that she would make it work this time. She did everything she and I could think of in milk-promoting management, but still did not come to a full supply. There were no medical conditions that could be a cause for inhibited milkproduction, nothing, just insufficient milk.
It was very hard for her and me to finally come to the conclusion that, yes, she was one of those very few women for whom breastfeeding doesn't work.
