Is hip pain her only symptom? How was osteoporosis diagnosed? Does she have
risk factors for vitamin D insufficiency (e.g., darker skin, higher
latitude, low sun exposure, etc.)? Has her vitamin D status been evaluated?
If not, why not? If so, what was her status? If vitamin D was low, what
treatment is she undergoing (e.g., vitamin D supplementation, increased sun
exposure)? Has her diet been evaluated by a dietitian? What about
weight-bearing exercise? Like carrying her baby regularly in a sling?
(After all, baby carrying is the original form of weight bearing exercise
for mothers. I often wonder if part of the calcium loss apparently
experienced by mothers during lactation isn't due to the fact that we have
strayed from our evolutionary heritage by using baby swings, strollers,
etc. instead of bearing the weight of our babies.)

I don't want to minimize the potential seriousness of a hip fracture.
However, it is appropriate for this mother to be thoroughly informed of the
risks of not breastfeeding her child (to her and to her child) so that she
can make a truly informed decision. Perhaps her physician would benefit
from some education on these risks, too.
