We get refered to this very often. I work in a hospital setting. I will hear them on the phone saying "the lactation nurse/breastfeeding person.... has come in". I do introduce myself as a lactation consultant but it doesn't matter. Initially I thought it was because they see me in scrubs but my partner dresses up and they still refer to her as the same. I am an RN although i never address that. 
I have come to think it is just because they are in the hospital. When in the hospital you will see either doctors or nurses and so i have to/must fit into one of those.
