Good morning, Lactnet.  This one is at the request of the patient and her
mother.  She is a 34 y/o primip, having given birth to VLBW twins (24 weeks
old DOB 8-19-03), the result of in vitro fertilization. One twin survived
the first week.  That survivor is now 7 lbs,  17 weeks since her early
birth, now passed her original due date.  The mother exceeded 25 oz./day EBM
by the third week. The mother is now producing an average of 74oz. a day and
has stored so much she is actually throwing some away! The baby was
discharged from the hospital finally two weeks ago. She donated a lot of
milk to the milk bank in Austin when she was in the hospital but now she
can't get there any more.

The strategies tried to decrease the milk production have been: cabbage
leaves, cabbage cream, taking pseudophed 60 mg q4, decreasing the length of
time she pumps but gets severely engorged ducts, tried to decrease frequency
of pumping sessions but has same problem.  The baby, now home, will NOT
latch directly.  I think it is clear to everybody involved that this would
probably be the answer to our oversupply problems.  The LC in the hospital
was marvelous, I understand, but apparently was not concerned about
decreasing the milk because she presumed that this problem would correct
itself as soon as the baby was directly feeding and directing the show.

I am not in the same city with this mother, so it's hard for me to second
guess all of the particulars.  She was in close contact with an IBCLC where
she is (Dallas) while they were in the hospital, but hasn't had much contact
since.  Her mother is a friend of mine and this query comes to you from the
mother of the baby via her mother.  Please send any and all suggestions to
me no matter how obvious they may seem. I will send them to her.
