I'm an IBCLC working at a teaching hospital.  We've had lactation services
since 1996, and now have four IBCLCs.  We don't give formula packs and our
Birthing Unit, NICU, Ped, PICU are all on board with supporting
breastfeeding.  We even have Pharmacy, Radiology, Surgery, and Anesthesia
calling for consults.  In other words we have made great strides in the past
six or seven years. Of course, we have our share of challenges as well.

Having said that, I'll share a story which took me completely by surprise.
On Friday I worked with a baby and mom having difficulty latching;  baby had
obvious tongue-tie.  I called the pediatrician and she arranged for the baby
to see an ENT the same afternoon.  I left the office feeling optimistic of
the chance of successful bf following a clip.

This morning a copy of the ENT's progress note greeted me.

"Baby has lingual frenulum.  Breathing fine thru nose.  Evidently has some
early problems nursing.  Nursing difficulties have nothing to do with
tongue.  Frenulectomy has risk infection and won't help him nurse better.
Can reevaluate as teenager."

I guess some gentle education is in order here.
