Well, 

To put "my" two cents in - and I don't think it's mine.  One of the
problems of going to so many conferences, reading Lactnet and Lacmat-L
(spanish lactnet) and reading at breakfast and dinner (I read the newspaper
at lunch) about breastfeeding is that I can't remember who said it or
when...

But... I have used the idea of putting the baby's nose to the nipple.  I
tell the  moms here to get the baby to smell the "pork chops" as pork chops
in Puerto Rico are usually very highly seasoned with garlic, oregano, salt
and pepper and then fried so that the whole neighborhood smells the (to me)
delicious smell.  It makes me hungry even when I've just eaten!  So -
especially for our sleepy NICU babies I ussually unwrap them, put them in
football hold skin to skin, then nose to nipple and express some milk -
usually colostrum.  This often gets baby to start moving the mouth a bit
and when mouth opens wide we put baby to breast lifting him ever so
slightly.  Now that I think about it, the neck must be slightly extended
though not extremely so.  The nose does touch the breast - but the much
more firm contact is the chin to breast.  

The big problem with all this positioning stuff is the need I feel for more
comfortable chairs, footstools  and LOTS of pillows so mom can relax and
let baby nip and nap...  (But didn't someone once say Americans are
Pillow-dependent when it comes to positioning.)  

How does this positioning sound to you?  It seems to work if anything will.
 It is easier for the LC too as she can sit next to the mom instead of
needing to lead over mom or (as I often do) kneeling in front of the mom. 
Oh that aching back or painful knees - now theres a use for another pillow!
