Yes I think the lack of bowel movements is a concern.
So there are several things to look for. You said the baby passed meconium
at birth. So I would want to know the frequency of feedings, the length of
feedings (are they very very short or very very long), the frequency and
character and color of the urine output, I would want to observe a feeding,
and I'd get a weight, and check for jaundice, and check how mom's breasts
are doing. I would ask about pacifier use, and feeds day and night. You said
the milk hasn't transitioned yet, I guess you mean her breasts are still
soft?
I would check the baby's abdomen because once in a while you'll get a sick
baby with a distended abdomen, but usually the baby is vomiting too. But
check just in case.
Anyway, just go back to basics, get all the questions answered. Hopefully by
this time, the milk has "come in" or "come down" as some say, and the baby
is getting loads of milk and has started having good output. If not,
supplementation may be needed. The reason we look for these signs (diaper
counts) is to tell us if there should be some intervention. Maybe just more
frequent feeds is all that is needed.
