Even in a hospital setting, it seems that one could say to a mom, "SInce the information we have on modifying caloric content in human milk contradicts this advice, I suggest we refrain from proceeding until we can get clarification".  Either way, I agree with Marianne--our obligation is to the family, not the physician. Always. 


As an anectdote. I have a friend whose baby recently had heart surgery at 5 mos. The NICU wanted to added HMF to her milk. Mom insisted on a creamatocrit and the results caused the NICU to back down. The range of calories from one mom to another can indeed be significant. 


I agree with Susan 100% here--this ped is behaving as in human milk is water and needs to be made into something worthwhile. If this mom has enough milk and is giving any AIM at all, I would be even more assertive with information--bc that alone is a risk that can never be undone or for which there is just no excuse.

I agree with Susan.  This wasn't simply boosting the calories a tad (20 to 
24). This was endangering the infant and the parents certainly had a right 
to know what could happen if they followed the advice.  Also how do we know 
for sure what he said and what mom thought he said??? You all know how that 
goes.  I think we are within our ethical realm to warn a parent of such a 
dangerous suggestion and to hold off until matter can be clarified with HCP.
