Lactose (carbohydrate) does not vary at different points during a feed,
neither does protein. Only the fat (and fat-soluble components) appears to
vary, depending on how full or empty the breast is at that moment. The
fuller the breast, the more the fat is diluted by the rest of the milk
components. Difference in fat from the start to the "end" of the feed helps
researchers calculate the storage capacity of the breast.

Left to their own devices, most mothers and babies figure this out just fine
without anyone telling them whether to offer both breasts per feed, just one
at a time, two feeds per side, etc. 

Chloe Fisher has long said "finish the first side first" which is still the
best general guidance around. 

Hartmann and colleagues in Perth, Australia have published many articles on
this including several in the Journal of Human Lactation.
