This is more related to the continued unrelieved headache than your milk
suppression question.
I am not trying to be an alarmist here but I HATE stories like this.
Does anyone have any suspicion that she might have had a small stroke from
pushing?  Yes it is rare. However, I have seen small strokes ignored and
blamed on an epidural turned  spinal and migraine before. And of course people
think mom is too young to have a stroke; so it's the last thing we think might
be causing the headache.
[My best friend had a small stroke after someone let her push incorrectly for
4 hours. She looked like she ran full speed, face first, into a truck for
almost a month. She still has short term memory problems and her youngest is
13.]
Did mom have HELLP syndrome with low platelets?  High blood pressure,
increased intra abdominal pressure, increased inter cranial pressure and low
platelet count can be a recipe for disaster.
Does this mom have broken blood vessels in her eyes, on her face and/or chest?
Petechiae start out like a pinpoint reddish purple rash and then get kind of
greenish--like an old bruise.  We often see Petechiae on baby's face after
birth, especially if the cord was tight around the baby's neck. Deep tissue
bruising can take up to a week to rise up to the surface where they are
visible.
If there are broken vessels below the skin, especially above the neck it is
safe to assume she might have blown out some vessels in her brain too. If she
does have broken vessels, try to talk to her doctor about an MRI just in
case--God forbid she's still bleeding in her brain.  I'd  worry a little about
the Reglan. It could make the situation worse by elevating blood pressure.
Do your best not to alarm the mom. I hope her HCP is someone you have good
rapport with. Otherwise, my experience is HCP's hit the roof at the mere
suggestion of a stroke as a cause of severe headache. It makes it very hard to
help the mom.
I remember something in Lawrence's book about ergot and milk production but
can't find it right now.
