Dear Lactnetters,

I would greatly appreciate any input especially regarding mother's low
weight profile affecting delayed lactogenesis.

Initial consultation mother 5 days postpartum after a home birth.  (Midwife
called me in as baby wouldn't suck on her finger and was fussy at breast.) I
found baby down in weight 10%.  Baby peeing 4 per 24 hours but total soiled
diapers only moster mec. at birth and 2 others day 3.  Midwife said cord
wrapped twice around baby's neck squeezed the mec. out of the babe.  Mother
103 lbs. prior to pregnancy, gained 30 lbs being 133 lbs. prior to delivery
and 5'7"-8" tall.  First pregnancy gained only 19 lbs. (no midwfe to help)
and supplemented early on.  Minimal swallowing at breast.  Baby sucked fine
on my finger but a bit weak.  Had mother pump with Lactina select just drops
pumped after a feeding.  Baby had been self attaching, did better at breast
with mother led attachment.  Lots of sucks for one swallow.

There is a lot more -  mother is Menonite and has 1 PKU relative on her side
and 2 on husband's.  Midwife detected arrthythmia on 3 separate occasions.
She went with Mom to FP MD he only noted tachycardia.  We'll have test
results on Wed.  Baby was dry and MD suggested sugar water prior to breast.

Day 6 evening still no stool, I started 1/3 ounce ABM per feeding via
p-syringe at breast.  Day 7 AM, mother is still only pumping drops (pumping
8 times per 24 hours), still no soiled diaper.  Midwife thinks low weight of
mother may be a factor.  I keep thinking of the women in third world
countries.  I was taught that 10% below normal body mass becomes a problem
with milk production and composition.  Midwife to see family PM Day 7 and
increase ABM if baby's weight loss is more.  Midwife also drawing blood for
maternal thyroid profile.  I think the important factor is reason for
mother's low weight more than low weight itself.
