A few years ago there was a story in the Israeli press……huge headlines:
Mother smothers baby while breastfeeding in bed.  I was called and
interviewed by one of the newspapers and I cited all the guidelines for
safe co-sleeping and mentioned that this just could not happen to a
healthy mother-baby dyad.  If the mother did not smoke, was not obese,
etc, then there must have been a problem with the baby. 
 After reading this in the paper, the parents requested a post mortem
examination of the baby and found that the baby had a serious heart
defect that had not been detected, and that the mom had not smothered
him, but that he had probably died peacefully in his sleep close to his
mother.  Of course this did not make the papers.  I only heard about it
months later from a friend of the mom.  She said that this mom would be
indebted to me forever for removing the guilt from her.
