In MY clinical experience, I have seen this disproportion a number of times,
but have always found that either the large size nipple shield was too large
anyway, so we may as well just wait for direct breastfeeding and pump, or
the large nipple shield still only got mostly the nipple, and there was
minimal milk transfer anyway, so pump...
I give lots of encouragement, and explain that even one month in the scheme
of many happy months/years nursing is worth the effort....
