One thing that bothers me about the transition hold is that moms remain with
their hand supporting the breast and their elbow sticking out like a chicken
wing. They are usually using a pillow so they are bent forward leaning
toward the baby. That could not be comfortable for long. I sometimes see
them when I am out and have to control myself not to go over and tell her to
remove her hand from the breast and hug her baby.

For this reason, I have stopped using pillows, have moms sitting back
normally and bringing baby to the breast, fold and flip the nipple ala
Rebecca Glover and once the baby latches, have mom replace her arm around
the baby. This way they end up in a sort of cradle position, where the baby
is cuddled in close to moms body, the baby's belly being under her other
breast ( I am working with newborns).

If the breasts are large and heavy, that is a different story, but most
women I work with are not obese, and do not have large heavy breasts.

Are any of you working in hospital situation trying the baby-led latch? We
have these idiotic gowns with the side split openings, which are designed
for women whose breasts come out of their waist. No skin to work with.
