How old is the baby, and has this been an issue since birth, or only recently?

I ask because babies will get wriggly at the breast if they are aware
of their elimination needs and are trying to figure out how to
eliminate without soiling mom - and of course they want to keep
nursing, so they get themselves into a wriggly dilemma!  Another sign
of this is popping on and off the breast.  Could also be a sign of an
uncomfortable wet or poopy diaper.

However, since you said the mom has had several courses of antibiotic,
that would be my first guess for the root cause.  If mom has had
antibiotics, some of it has gotten to the baby, too, so their gut is
not in tip-top shape either.  Couple that with mom passing food
proteins into her milk that normally wouldn't be there, and you can
get an unhappy baby.
