Thanks to everyone for the wonderful comments/info on the foremilk/hindmilk
issue.  I loved Pam Hirsch' statement: "It's more important that they relax
and enjoy nursing their babies, rather than get hung up on whether they have
enough hind milk." 

I work with a wonderful population of highly motivated, highly educated,
older, professional-career moms who tend to treat breastfeeding as a project
to be researched and implemented methodically.  As a result they approach
hindmilk with the zeal of a crusader looking for the Holy Grail.  Many of
them end up underfeeding their babies, with consequent low milk production,
as they try to "empty" a breast which, as was pointed out, is a faulty
concept.  Like Pam and others who wrote in, I try to encourage moms to focus
on enjoying the baby at breast and just keep an eye on swallowing (if
they're not swallowing who cares what kind of milk is in there!).  My mantra
is "how did we breastfeed for thousands of years?!"
