Have you checked to see if the frenulum is tight?  This would explain this
type of a problem.  How much clicking is there when baby is on deeply?  Can
baby maintain a deep latch w/o breaking suction and clicking?

What kind of palate does this baby have?  What kind of MER?  Is it fast with
lots of milk coming all at once?  Is mother having problems with plugged
ducts or breast infections?
