I like Nina's thoughts on the action plan of making empowerment of the mother more
explicit. Really she is the implementer of any changes to practice and this should be
recognized. This is another step forward from "care plans" which are even further down the
totem pole of empowerment. For years, I have been modifying mother and baby "care
plans" from what my mentors used and it dawned on me that really, these are not "care"
plans for the mother and baby because as an IBCLC I am not there caring for them --- I am
observing and advising them on ideas about how to improve practices and I really need to
engage the mother to help her determine what will work best for her to take responsbility
to improve her practices. Nina, I hope you sent your comments to ILCA.

Following the WHO code SHOULD be in the standards of practice. A scope of practice is
what we CAN do --- while standards are more specifically detailed about how we should be
conducting ourselves including ethics. And I most wholeheartedly support following the
WHO code for IBCLC "STANDARDS" of practice.

Best, Susan
