I was in fairly regular contact with David Porter several years ago
as I was learning the Concord Sonata.  The discussion of the orchestra
version often came up.  It seemed to me that he was largely doing a
combination of piecing together, cleaning up, and making very meticulous
"judgement calls" as to some notes, instrumentation, etc.
