The entrance comes with a plastic plug to close it off.  I also use a vent box over all my inner covers.  Basically, an empty super box.  Some have screened vent holes, some don't.  I think it acts like an attic, serving as a place for hot air to rise in the summer, and as a buffer in the winter.  It's also an overflow 'safety valve' in case a hive booms while I am away - the bees have somewhere to go besides the nearest tree.  And, it protects the 'over-the-inner-cover-hole' feeders I use.
