I would have used 1:1. 2:1 is pretty thick and does not take the place of
nectar as well as 1:1 does.


I have used plastic frame/foundation as well as wooden frame/plastic foundation with no problem.

It does sound like you had a violation of bee-space but that is at best a guess on my part.
