For my operation, I use grocery sacks (paper).  I tear them open at the seams where they have been glued, then cut the big sheet to standard letter size sheets.  For the label, I use my own pattern but designed so that I can fold the label in half so that it has four pages.  I get eight to ten labels per page.  I dis-remember which.  I then use an ice pick (careful here) to make a hole in the top folded corner.  To attach the label to the jar, I use thin baling type hemp cord (about 1/8" in diameter).  Used with regular canning jars, it gives it the real rustic look.  With the four pages (first outside page is my apiary identification and address) I can insert all kinds of information including instruction on how to reliquefy crystallized honey and web addresses on the back page for honey recipes.
