Ohio - $5/apiary/year.
I am not certain where/how this fee is disbursed, but like to think the funds support the ODA apiary program (http://www.agri.ohio.gov/divs/plant/apiary/apiary.aspx).  I am not aware of any real 'services' provided directly to beekeepers.  They used to have a couple of state inspectors who hands-on assisted county inspectors, but now seem to have assumed more of a reporting/coordinating role.  Inspectors, in counties that have them, are typically funded by the county commissioners.
