Is the 40% efficacy not calculated after the series, and include all 
three treatments?

Also, the calculation of efficacy is a curious practise, with an actual 
significant positive efficacy sometimes ascribed to the controls.

That blew me a way since the controls should be the zero reference IMO, 
but apparently that is not how it is done.

Anyone care to explain?
