In my experience there is usually either something wrong with the bees or the beekeeping if that happens.

Sick bees or puny clusters, it seems, cannot generate the energy to expand the cluster as they should.

The beekeeper is implicated if the feed is so far off and to the sides that the bees are unable to reach it -- or such poor quality that on reching it, the bees are unable to utilise it.
