Classic signs of full blown Nosema ceranae, and often of CCD.
 
 
The advice to clean out feeder, feed fumagillin is same as I  would give.  
But, at this stage, bees seldom take syrup, so you either have  to drench or 
do something to get them to eat the drug.  We have found that  a dash of 
Honey Bee Healthy at times is enough of a stimulant to get even sick  bees to
