So here is a further question: Of the original sightings of the malady, 
very early on, before compensation was available, what proportion may 
have been true CCD (with the full set of four symptoms- loss of 
workers, high brood to worker ratio, no robbing, no attacks from pests) 
vs. "CCD" (the longer more diffuse list of lookalikes)?

Some of the pictures of colonies presented at the time with the full 
set of symptoms were quite shocking.  Would definitely be good to have 
a better feeling of how prevalent this extreme form was.

Many of us who never saw this extreme form were skeptical.  Probably 
still are.  But a negative is hard to prove, and what some reported 
(and the pictures) do show something happened.
